Creating a slave file

When one makes the discussion to take a slave there are many pieces
of information that every Master/Mistress/Dom/Domme should ask for.
I have talked to Master’s who have no idea what medication, if any,
their slave takes, and others who have never asked about education
or even criminal records. Most of the time lack of information can
be at the least inconvenient at most it can be dangerous. Let me
ask just a few questions and see how much you know about your
slave/submissive, then you can judge if you need to have a slave
file or not.

Does your slave have a criminal record, if so for what?
-
Has your slave ever used any form of illegal drugs, if so
what and when?
-
Has your slave ever had any form of major surgery, if so what and when?
-
Does your slave take any medications, if so...
What type of medication does your slave take?
What is the medication for?
How often does your slave need to take the medicine?
What pharmacy does he/she use to fill their prescription?
Are their any medications or foods that may react badly with
the prescription?

What is the name of your slave’s doctor?
-
Does your slave have insurance, if so what is the name of
the insurance company and what type of coverage does he/she
have?
-
Does your slave have any allergies, if so to what, and what
normally happens during an allergic reaction?
-
What is the license plate number of your slave’s car?
-
Who is your slave’s closest relative and what is this person’s
phone number?
-
What level of education does your slave have?
-
What is your slave’s social security number or driver’s
license number?
-
Has your slave been tested for Sexually Transmitted
Diseases including HIV/AIDS, if so when and what were
the results?
-
What different jobs has your slave held in the past five
years, and how long was he/she at each, also why did he/she
leave?
-
What different groups does your slave belong to, within
the lifestyle as well as outside the lifestyle?
-
Who was your slave’s last sexual partner, also what was
their relationship?
-
Has your slave ever gone through counseling and if so
what are the details surrounding this?
-
Does your slave have any phobia, such as to snakes,
heights, closed in places, if so what is the phobia,
and to what degree are they affected?
-
How much money does your slave owe and to whom?
-
Has your slave ever won an award if so for what?
-
Are yours slave’s parents living or not, also does
your slave have siblings, if so what are their names?
There you have it 20 questions, no problem right...
could you answer all of them? If not then you need to set
up a slave file. There is a lot of information out there
that unfortunately may be necessary to have one day. I know,
I know... most people say "My sub is only here on the
weekends so why do I need to keep a whole bunch of useless
information sitting around?" The answer is simple...
me give you an example.
Example 1: You go out to dinner with your slave and she orders
the General Toa’s Chicken, after about 3 bites she starts clearing
her throat, 5 minutes later she is lying on the floor of the
restaurant unconscious. It isn’t until after you are riding in
the back of the ambulance watching the paramedic put IV’s into
her arm that you are informed she is having an allergic reaction,
and it isn’t until you have spent 4 hours in the emergency room
that you realize she is severely allergic to peanuts.
Example 2: You get a call at 3:00am; the officer found your
number in her wallet and wants to know if you know Jane Doe.
There has been a car accident and she is in a coma at the
Regency Hospital. When you get to the hospital you are plummeted
with questions, does she have any living relatives, does she
have insurance, is she allergic to penicillin, how about morphine,
does she have a living will? Question after question are asked,
you do your best and set in a daze while answering the endless
barrage of questions. Finally you are allowed to see her, it
doesn’t look good, they suggest you make some calls to relatives
and friends just in case. You spend the next several hours rummaging
through her stuff looking for information, phone numbers of friends
and family members.
Example 3: Your slave is driving you to work and is pulled over
for a taillight being out. The officer runs his license through
and finds out there is a warrant out for his arrest. Your slave
is cuffed and placed in the back of the patrol car and you spend
the next several hours getting things straightened out. You find
out that your slave wrote some bad checks about 3 years ago and
failed to show up for a court date. After $1800.00 in fines, and
$693.00 in paying the checks, as well as a new trial, community
service and a sentence to 3 yrs probation, things are finally
taken care of.
These are just three examples of some of the problems that could
very well occur. How does one avoid such things, well unfortunately
there may be no way to avoid accidents, or problems. However one can
be prepared, in example one it would have only taken a simple question,
"Are you allergic to anything?" In example two it takes a
little time and effort to get the contact information from your slave
and have it written down as well as the vital medical information. In
example three well let’s just say that some questions are really stupid
not to ask, but unfortunately sometimes they get overlooked. So now
that you realize that there is some valuable information that does
need to be shared the next question should be is what questions
should I ask? I have made up a general list of requirements that
I have for my slaves/submissives, understand that these are just
some of the basic, there will be other questions or requirements
that you want to add.

Copy of birth certificate, or copy of driver’s license and
social security card.

Copy of high school or college degree, and/or copy of
transcripts of any college attended. Also any certificates
for classes taken.

Copy of marriage license, divorce decree, or annulment
(where applicable).
-
Copy of car registration, and insurance.
-
Recent photograph, preferably one in color as well as
one in black and white.
-
Full medical records, if not available from doctor,
I request a complete disclosure of medical history be
provided.

A complete listing of all medications taken, when, and
for what.

A complete listing of Doctor, and Dentist information,
don’t forget that they may have specialists, like gynecologist,
optometrist, or chiropractor. You want the names, addresses
and phone numbers of each one... it might also be helpful to
get a little history on each as well.

A list of any allergies, their reactions, and treatments.
-
A copy of the most recent HIV test results. Should be less
than a year old.
-
A list and explanation of any phobias that the slave has
as well as how they would react.
-
Psychological records, for any counseling, therapy, or
psychological evaluations that have been done, if records
are not available then I request a full disclosure and
history provided.

Criminal records, I request a complete disclosure of
any legal action, from a parking ticket to a class D
Felony and every thing in between.

Complete disclosure of all relationships, this should
include everything from the first sexual experience up
to and including the present. Details are very important
so the more information the better. Sometimes all the
slave/sub can do is put down a first name or "the
guy from work" but just be as thorough as possible.
This may be very important if the issue of HIV/AIDS,
other sexually transmitted diseases, even pregnancy
comes into your relationship.

An up to date resume.
-
A submissive play list, or check list.
-
A complete list of all Dom/Dommes that he/she has
served under or significant experiences within the
BDSM lifestyle, as well as details on the relationships
or scenes.

A complete list of any formal training within the BDSM
lifestyle.
-
A complete list of addresses that the slave has had in
the past 5 – 10 yrs.
-
A complete list of relatives, and friends to be contacted
in case of emergency, make sure you get full names, phone
numbers and addresses.
-
Complete financial disclosure, such as bank used, checking
and savings acct information, credit card information, and
all school loan or car loan information, it might also be
useful to get a copy of the previous years tax returns.

A complete list of groups, associations, and memberships.
-
A complete list of email addresses, as well as the
passwords for the accts.
-
References if available.
-
Church affiliation.
-
A copy of a living will, or durable power of attorney.
I know all this seems like a lot but honestly when it comes down
to it the more information the better. There are a lot of pieces
of information that I put in the file as I go, things I have found
out about the slave that I think are important. Things like...

Favorite flower
-
Favorite gemstone
-
Favorite color
-
Shoe size
-
Dress size
-
Pant size
-
Underwear size
-
Shirt size
-
Ring size
-
Favorite type of food (Mexican, Italian)
-
Favorite pet
-
Collections
-
Hobbies
-
Favorite music
-
Favorite author
-
Favorite actor
-
Favorite actress
-
Favorite movie
-
Favorite alcoholic beverage
-
Favorite beverage
-
Religion
Lots and lots and lots more.
Everyone puts different information in, and sometimes the files get
pretty big. However, when ever I need the information... whether it
is to make a life and death decision or to pick out a birthday present
I have the information.
There are a lot of reasons to make such a file, but I can only
think of two reasons not to make it either you don’t care, or
just pure laziness. Sure it takes time and sure it takes a lot
of work, but believe me it is worth it.
I have put together a file about myself with much of this information
in it for my slaves/submissives and believe me it gives them a
feeling of security, they know I am serious about the relationship
and that I am willing to take the time to give them that piece of mind.
